All About Aging is a Care Management practice started in 2006 that serves Polk and Highlands Counties. Our care managers help older individuals and their families enjoy their later years by assisting in the navigation through the aging process.  We assess the current state of the client, develop and implement care plans, and provide ongoing support and monitoring. This includes physician visit management, hospitalization/emergency room management, assisting with care transitions, and moving into facilities or back home to be with family.  We are always assessing our clients’ current state and advocating for them so they receive the best care for their situation, often recommending medication changes, getting orders for home health, or facilitating the acceptance of caregivers in the home.
